Dell Medical School Comprehensive Diagnostic and Treatment Evaluation ReportSigned February 3, 2025, by seven Dell Medical School faculty. Contains the Chief Complaint documenting financial coercion. Contains the WAIS-IV scores mischaracterized as "above average." States "no evidence of any mania."
September 2024 Laboratory PanelVibrant labs documenting CD57 immune suppression, Complement C4a systemic inflammation at 2.4x reference range, and Aspergillus fumigatus mycotoxins at approximately 8x reference range — three months before Dell diagnosed Somatic Symptom Disorder.
December 12, 2024 Lab Order Containing ICD-10 F31.9 (Bipolar Disorder, Unspecified)Obtained through federal OCR intervention. Never disclosed to patient during evaluation. Contradicted by the final report itself. Triggered a UT Austin research recruitment invitation in January 2026.
Sister's April 23, 2025 Written ConfirmationLetter in which patient's sister confirms sending a letter to Dr. Erin Logue during the evaluation, describes it as "meant to be confidential," and acknowledges the content could be "hurtful."
UT Health Austin HIM Response, November 12, 2025Written statement from UT Health Austin's Health Information Management department stating no such document exists in patient's file. Directly contradicts the sister's written confirmation.
Altered and Backdated RecordsTwo versions of medical records in patient's possession. Final report of February 3, 2025 includes NPD, no mention of Adjustment Disorder. After patient challenged findings February 4, 2025, records were altered: NPD removed, Adjustment Disorder inserted backdated to January 15, 2025.
Family Email Chain, November 2024Emails between patient, parents, and sister documenting the financial coercion in the family's own words, including the mother's written statement: "I'm afraid that disability check is not going to go very far."
OCR Correspondence — First and Second ComplaintsFirst OCR complaint required federal intervention in 2025 to force UT to produce records. Second OCR complaint closed in December 2025 under the current federal administration with a letter stating OCR would not investigate.

UT Austin Ethics Office: Complaint filed May 30, 2025. No substantive response.
Texas Medical Board: Complaint filed November 5, 2025 against Nemeroff (License R9406). TMB declined to investigate.
HHS Office for Civil Rights (First): Filed 2025. Required federal intervention to compel UT to produce records.
Cure Demand to UT Health Austin HIM: Sent January 24, 2026. No response.
HHS Office of Inspector General: Filed February 10, 2026. No response.
SACSCOC Formal Accreditation Complaint: Filed February 2026. Pending.
HHS Office for Civil Rights (Second): Filed November 2025. Closed December 2025 with statement that OCR would not investigate.

Key Public Sources for Corroboration
Senate Finance Committee investigation of Dr. Charles Nemeroff (2008): Public record. Chairman Chuck Grassley. Documented approximately $1.2 million in undisclosed pharmaceutical company payments during NIH-funded research at Emory University.
Judge Kathleen M. Williams ruling on IRS-Trump settlement (2026): Federal Southern District of Florida. Ruled the settlement "obviously collusive" and pursued "for an improper purpose." Referred attorneys for sanctions.
Judge Kathleen Williams ruling and DOJ settlement documents (2026): Southern District of Florida. Found the parties "worked in tandem and were never actually adverse." Justice Department press release announcing the $1.776 billion Anti-Weaponization Fund, and the order barring the IRS "forever" from existing tax claims against the President and his family.
Senator Cornyn public statements (July 2026): On the principle that written documentation controls regardless of subsequent verbal characterization.
